Check-out Instructions

If you would like to check out one or both of the test kits, please contact Claudia Guy to coordinate (contact info below). Please return the test kit within 2 weeks of checkout so it is available to other municipalities, if needed. Before returning the test kit(s), please fill out the inventory form (which will be available in each test kit), so we can keep track of when reagents or other supplies need to be ordered.
